The Importance of Long-Term Care Planning
For most of us, life is busy and extra time is rarely available – especially when caring for an aging loved one. Understandably, many adult children and spouses lack the time, resources, and experience needed to navigate these complexities and provide the level of care their loved one deserves. Doing so becomes even more difficult as conditions progress and demand a level of care and expertise that few family members are prepared to provide.
To help, Unicity Healthcare offers long-term planning services from experienced, caring Senior Advisors. As part of a team of registered nurses and licensed social workers, Senior Advisors form trusting relationships with clients and their families to create comprehensive long-term care plans that enhance comfort, safety, and peace of mind.
In-Depth Geriatric Case Assessment
The long-term care planning process begins with a comprehensive geriatric care analysis. During the initial interview, a Senior Advisor will evaluate a number of elements to make personalized recommendations for existing needs and future concerns. The full geriatric case assessment includes:
- Physical health review that includes medical history, current medications, and mobility
- Depression screening
- Assessment of home safety and areas for improvement
- Review of insurance, financial, legal, and government program considerations
- Daily living activities, such as driving ability, hobbies and interests, and more
- Client and family preferences for care and residential options
When this analysis is complete, you will receive a detailed assessment of your loved one’s current requirements and projected needs, as well as suggestions for informed solutions to address them.
